The Employee Evaluation Form is a systematic tool of Achieve Without Borders, Inc. for the overall assessment of probationary employees and appraisal of regular employees.
The evaluation process is composed of three to four phases (four for team members with subordinates; three for team members without subordinates) wherein the Employee Evaluation Form will be distributed to different individuals for each phases. For the first phase, the employee for evaluation will receive the Employee Evaluation Form to conduct self-appraisal. Once the self-appraisal is done, the Employee evaluation form will be forwarded to the Supervisor for the second phase of the evaluation. The third phase will follow as soon as the supervisor accomplish the form; The Employee evaluation form will then be forwarded to two team mates of the employee being evaluated for their round of evaluation. The fourth phase will depend on the role of the employee being evaluated. If ever he/she has a subordinate, the fourth phase will commence and the Employee Evaluation Form will be provided to his/her subordinates.
The data from the different phases of the evaluation will be consolidated by the Human Resources for the finalization of the result. A recommendation will be raised to the supervisor once the result is available and inform the employee being evaluated.
Please take note that the evaluators for the third phase will be kept confidential- the same applies if the subordinates of the employee being evaluated are numerous.

Does the employee provide or exceed the required number of outputs?
The extent to which the employee's output is well executed, thorough, effective, accurate, and meets the expectations of the job.
Are the outputs of the employee acceptable in the standards of his/her job?
The extent to which the employee knows and demonstrates how and why to do all phases of assigned work, given the employee seeks counsel.
The manner in which the employee responds to supervisory directions and comments, and how the employee gets along with other individuals in the company.
The extent to which the employee claims ownership of his/her role and behaves professionally - which in turn makes him/her dependable.
The extent to which the employee is self- directed, resourceful and creative in meeting job objectives; consider how well the employee follows through on assignments and modifies or develops new ideas, methods, or procedures to effectively meet changing circumstances
The extent to which the employee demonstrates the ability and willingness to accept new/more complex duties/responsibilities.
The capacity of the employee to effectively lead and guide his or her subordinates, team members, and team.
Can the employee lead another team member in achieving a common goal?
Can the employee motivate his/her subordinates and team mates in achieving their goals?
